25 Again Member Communications

Dear Valued 25 Again Members,

We place great importance on the quality of our patient communication. Ensuring your ease of access to us, tailored to the nature of your inquiry, is our priority. Please refer to the information below to ensure your needs are promptly and accurately addressed.

Provider Cell Phones
Please reserve contact with your Provider's cell phone for urgent medical concerns only. Matters such as scheduling, billing, or refills should be directed to the appropriate phone number or extension as detailed below.

Prescription Refills
Please contact your pharmacy; they will send us a refill request.

  • We suggest putting your prescriptions on auto-refill or auto-notice (if a controlled substance), so your pharmacy will send us a request automatically whenever your script is due. This can be done through your pharmacy.

Questions About Your Prescriptions and Medical Care

Contact your care team directly by calling your clinic and selecting the appropriate option from the phone menu (see below).

PLEASE NOTE: Our Medical Assistants are busy attending to patients throughout the day. If they are unable to answer your call, please leave a voicemail, and they will promptly return your message.
